9 to 11 Weeks Old

• First Infectious Tracheal Bronchitis – Bordetella *If boarding, puppy obedience class, or dog park
• First Lyme Disease Vaccination
• Heartworm & Flea-Tick Preventative – 2nd Dose

12 to 14 Weeks Old

• Third Combination Vaccination – DA2PPLC
• Second Lyme Disease Vaccination
• Second Infectious Tracheal Bronchitis – Bordetella *If boarding, puppy obedience class, or dog park
• Heartworm & Flea-Tick Preventative – 3rd Dose

15 to 17 Weeks Old

• Fourth Combination Vaccination – DA2PPLC
• Rabies 1-year Vaccination
• Heartworm & Flea-Tick Preventative – once a month all year

4 to 6 Months Old

• Pre-Anesthetic Panel
• Spay or Neuter Surgery

Annually (1-6 years of age)

• Comprehensive Wellness Examination & Dental Health Evaluation
• Combination Vaccine Booster – DA2PPLC
